Role Overview
As a Hospice Aide, you will provide personal care under the guidance of a registered nurse. This position requires travel between patient homes and facilities, and we offer mileage reimbursement. Initially part-time, this role has the potential to grow as our patient census increases. Morning visits typically begin around 7 AM, with hours lasting from 6-8 hours depending on the number of visits scheduled.

Key Responsibilities
- Document care provided in accordance with organizational policies.
- Notify the Case Manager of any changes from the Care Plan.
- Safely utilize Durable Medical Equipment.
- Take vital signs as outlined in the Hospice Aide Care Plan.
- Collaborate with team members and actively engage in staff meetings.
- Monitor and care for the patient’s skin to prevent breakdown; report any concerns to the Case Manager.
- Assist with patient mobility, transfer, and range of motion, as specified in the Care Plan.
- Offer respite for family members and caregivers when onsite, as needed.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Engage in Interdisciplinary Group (IDG) meetings as appropriate.
Qualifications
- Must meet training requirements as mandated by the state of Utah.
- Minimum one year of experience as a CNA, hospice aide, or home health aide.
- Must be at least 18 years old.
- Valid driver's license with an insured vehicle in good working condition.
- Excellent communication skills, both verbal and written.
- Able to work collaboratively in a team environment, as well as independently.
