Ovation Hospice is looking for dedicated and caring Hospice Aides to make a difference in the lives of our patients through morning visits. If you are passionate about providing quality care and support, we want to hear from you!

Role Overview

As a Hospice Aide, you will provide personal care under the guidance of a registered nurse. This position requires travel between patient homes and facilities, and we offer mileage reimbursement. Initially part-time, this role has the potential to grow as our patient census increases. Morning visits typically begin around 7 AM, with hours lasting from 6-8 hours depending on the number of visits scheduled.

Key Responsibilities

  • Document care provided in accordance with organizational policies.
  • Notify the Case Manager of any changes from the Care Plan.
  • Safely utilize Durable Medical Equipment.
  • Take vital signs as outlined in the Hospice Aide Care Plan.
  • Collaborate with team members and actively engage in staff meetings.
  • Monitor and care for the patient’s skin to prevent breakdown; report any concerns to the Case Manager.
  • Assist with patient mobility, transfer, and range of motion, as specified in the Care Plan.
  • Offer respite for family members and caregivers when onsite, as needed.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Engage in Interdisciplinary Group (IDG) meetings as appropriate.

Qualifications

  • Must meet training requirements as mandated by the state of Utah.
  • Minimum one year of experience as a CNA, hospice aide, or home health aide.
  • Must be at least 18 years old.
  • Valid driver's license with an insured vehicle in good working condition.
  • Excellent communication skills, both verbal and written.
  • Able to work collaboratively in a team environment, as well as independently.
